iCare Implementing Care Plan

We understand that putting care in place can be very daunting, so we aim to make this first step simple and seamless for you and your loved ones.

Our initial care planning assessment is when we first really get to understand the daily or weekly support requirements, as this is vital when planning the right level of care.

Care Implementation - Assessment Visits

Implementing care for either yourself or a loved one is straightforward and we do this every day. Our team will visit you at home or in hospital and carry out a comprehensive assessment, which will ensure that they get to know you well enough to build a professional and caring relationship from the very beginning.

The assessments will observe your needs, lifestyle and choices, enabling us to create a detailed person-centred care plan. During our assessment we will also review any moving and positioning equipment in place, along with the layout of the property, to ensure that all the elements of our care delivery are 100% safe for you and the care team.

We always welcome and encourage a next of kin, relatives, social workers and health professionals to be present at these assessments, to ensure we gather as much information as possible.

Care Planning

The information we gather during our assessment will help us write your comprehensive person-centred care plan.

The care plan will cover your life so far, plus it will provide care staff with a very detailed record of your needs and preferences, so your care can be delivered just the way you like. After all, you are the most important person when we are delivering the care.

Once we have written the care plan we will visit you at home and ensure that we have got it right. If you feel that you need to change how and when something is done, our team is on hand to update the care plan where required. We are just a phone call away and will always update your care plan as and when a change takes place.

Care Review - “Because we care, we know”

We review your care needs on a regular basis, at least every three months or sooner if the need arises. This enables us to keep in close contact and ensure that all your needs are being met on an ongoing basis.

We carry out quality assurance phone calls every two months to ensure that your care is being delivered just the way you like it. If at anytime you would like your care to be reviewed, then just give the team a call and they will arrange to visit you at home.
